Home Care Physician (MD/DO) - Hybrid (Medicare Advantage)

Location: Field-based in Georgia; hybrid schedule with 3 days in the field and 2 days working from home (1 virtual health day and 1 half virtual care/half administrative day).

Salary: $220,000–$240,000 USD annually, plus a performance-based bonus program.

What you will do

• Provide patient-facing medical care and comprehensive assessments in members’ homes across Georgia.
• Manage a panel of medically complex Medicare Advantage members and remain accountable for clinical outcomes.
• Lead post-discharge and transitional care visits, manage chronic and acute needs, address medication concerns, and close care gaps.
• Identify changes in clinical status early and intervene proactively to reduce avoidable complications and readmissions.
• Build trusted relationships with members, caregivers, and families while coordinating care with nurses, pharmacists, advanced practice clinicians, specialists, and community partners.
• Use Clover Assistant and other clinical technology tools to guide evidence-based decisions, document care, and support continuity.

Requirements

• MD or DO degree.
• Board certification in Internal Medicine, Family Medicine, Emergency Medicine, or a related primary care field.
• Active New Jersey medical license, DEA registration, and CDS registration required.
• Strong clinical judgment and experience managing chronic conditions in older adults, including medically complex, frail, and socially vulnerable patients.
• Ability to practice independently in a field-based environment with strong documentation, communication, and care coordination skills.
• Proficiency with clinical technology and curiosity about using data and tools such as Clover Assistant to improve care.

Nice to have

• Geriatrics experience or fellowship training.
• Experience in home-based care, value-based care, population health, complex care, or Medicare Advantage.
