Digital Care Advanced Practice Provider (NP or PA) - Responsive Care

Location: Remote USA; must be located within one of the contiguous 50 states. Expected coverage is four 10-hour shifts per week from 8am–6pm MST/PST; EST/CST schedules may be considered.

Salary: $120,000–$155,000 base annually, plus a performance-based bonus of $10,000–$20,000, for total compensation of $130,000–$170,000.

What you will do

• Deliver empathetic clinical care through real-time and asynchronous chat and other responsive care channels.
• Manage concurrent patient conversations, symptom assessments, medication questions, lab results, follow-up, and accurate EHR documentation.
• Guide holistic medical management using the 5M framework: Mind, Mobility, Medications, Multicomplexity, and Matters Most.
• Coordinate care with primary care providers, specialists, pharmacy teams, family members, and caregivers.
• Escalate patients to telephone, video, in-person, urgent, or emergency care when chat-based care is insufficient or unsafe.
• Improve digital workflows, standardized responses, escalation pathways, quality standards, and patient outcomes.

Requirements

• Advanced Practice Provider with at least three years of experience in primary care, house calls, internal medicine, family medicine, geriatrics, palliative care, and/or urgent care.
• Active, in-good-standing RN and APRN licenses for Nurse Practitioners, or an active, in-good-standing Physician Assistant license for Physician Assistants.
• Ability to obtain licenses in requested states within 90 days of the hire date; additional state licensure may be required.
• Ability to review AI-generated responses for clinical accuracy and safety.
• Strong clinical judgment, documentation skills, patient communication, and ability to manage multiple asynchronous conversations.

Nice to have

• Virtual care or telehealth experience.
• Experience in real-time, queue-based, or high-volume telehealth environments.
• Experience collaborating with interdisciplinary care teams and coordinating care remotely.
• Experience supporting complex or vulnerable patient populations in digital-first settings.
