CS Connect - Primary Care Physician-Weekday (MD/DO BCFM), CA, 100% Remote (AI)

Location: 100% remote from the United States; active California medical license required. Monday and Friday availability is required, with flexibility for starting hours between 7:00 am and 12:30 pm PST.

Salary: $250,000–$300,000 USD annually.

What you will do

• Deliver preventive, acute, and chronic care through fully remote video visits.
• Manage a dedicated panel of patients and build longitudinal primary care relationships.
• Refer patients to specialists or acute care services when clinically indicated.
• Support population health initiatives, including annual wellness visits, care gaps, transitions of care, and risk capture.
• Provide post-discharge support to improve treatment-plan understanding and reduce readmissions.
• Evaluate clinical practice continuously to maintain adherence to guidelines and best practices.

Requirements

• Board certification in Family Medicine or Internal Medicine.
• 3+ years of post-residency experience, including at least 2 years in an in-person primary care setting.
• Experience with virtual visits and the EPIC EMR.
• Active California medical license required; IMLC preferred.
• Eligibility or current enrollment as a Medicare provider, with a clean background and medical malpractice history.
• Availability for 40 hours per week, including 36 clinical hours, and capacity to handle 3+ cases per hour.
