Weekend Digital Care Advanced Practice Provider (NP or PA) - Responsive Care

Location: Fully remote within one of the contiguous 50 states of the USA. Weekend coverage is required, with a schedule of 3x12-hour or 4x10-hour shifts.

Salary: $120,000–$155,000 base annually, plus a performance-based bonus of $10,000–$20,000 for total compensation of $130,000–$170,000.

What you will do

• Deliver empathetic clinical care through real-time and asynchronous chat and other digital care channels.
• Manage concurrent patient conversations, providing assessment, guidance, follow-up, documentation, and resolution or handoff.
• Support symptom management, chronic disease care, medication questions, lab-result reviews, and broader medical management using a holistic framework.
• Coordinate care with primary care providers, specialists, pharmacy teams, caregivers, and other interdisciplinary partners.
• Escalate patients to telephone, video, in-person, urgent, or emergency care when chat-based care is not appropriate.
• Improve digital workflows, standardized responses, escalation pathways, quality standards, and patient experience.

Requirements

• Advanced Practice Provider with at least three years of experience in primary care, house calls, internal medicine, family medicine, geriatrics, palliative care, and/or urgent care.
• Active, unrestricted RN and APRN licenses for Nurse Practitioners, or an active Physician Assistant license for Physician Assistants.
• Ability to obtain and maintain licenses in additional requested states within 90 days of hire; hirify.global covers licensing costs and provides administrative support.
• Ability to review AI-generated responses for clinical accuracy and safety.
• Ability to work weekend coverage in a 3x12-hour or 4x10-hour schedule.
• Strong clinical judgment, documentation skills, patient focus, and adherence to privacy and telehealth regulations.

Nice to have

• Prior virtual care or telehealth experience.
• Experience in real-time, queue-based, or high-volume telehealth environments.
• Experience coordinating care remotely with interdisciplinary teams.
• Experience supporting complex or vulnerable patient populations in digital-first settings.
