Complex Care Outreach Coordinator (Healthcare)

Location: Remote (USA). Must be willing to work 8:30 AM - 5 PM MST/PST.

Salary: $25.00 - $30.00 hourly

What you will do

• Conduct outreach via telephone and SMS to educate members on clinical programs and essential screenings.
• Schedule and thoughtfully adjust appointments for multidisciplinary care team members.
• Provide inbound call support and routing for patients with clinical or administrative inquiries.
• Collaborate with physicians, NPs, and case managers during daily coordination huddles and patient rounds.
• Investigate and resolve member questions regarding benefit offerings.
• Document accurate member needs and requirements within the internal system.

Requirements

• Bilingual Spanish speaker.
• Minimum 3 years of healthcare experience in a customer or patient-serving role (e.g., Care Coordinator).
• Must be available to work Monday-Friday, 8:30 AM - 5 PM PST/MST.

Nice to have

• 1-2 years of call center experience.
• Experience with health insurance, specifically Medicare Advantage.
• Knowledge of medical terminology and HIPAA regulations.
• Proficiency in Google Suite (Drive, Docs, Sheets, Slides).
