Bilingual (Spanish/English) Everyday Care Advanced Practice Provider (NP or PA)

Location: Fully remote within the contiguous 50 states of the USA. Monday–Friday, 8-hour shifts; 8am–5pm MST/PST preferred, with EST/CST hours considered.

Salary: $120,000–$155,000 base, plus a performance-based bonus of $10,000–$20,000; total compensation range of $130,000–$170,000.

What you will do

• Conduct focused virtual assessments for patients with hypertension, hyperlipidemia, coronary artery disease, diabetes, and other chronic conditions.
• Develop diagnoses and individualized treatment plans covering medication management, lifestyle changes, and monitoring.
• Manage prescription refill queues and centralized results workflows, including review, triage, and follow-up on laboratory and diagnostic results.
• Use digital portal chat and AI-assisted prescribing and triage tools while providing feedback to improve clinical workflows.
• Identify risks, comorbidities, and treatment contraindications; counsel patients on adherence and side effects.
• Collaborate with PCPs, specialists, pharmacy, nursing, social work, families, and caregivers to coordinate patient care.

Requirements

• NP or PA with at least three years of outpatient clinical practice experience.
• At least two years of primary care or relevant subspecialty experience focused on hypertension and lipid management.
• Bilingual Spanish and English proficiency required.
• NPs must hold active, unrestricted RN and APRN licenses; PAs must hold an active, unrestricted Physician Assistant license.
• Must be located within the contiguous 50 states and able to obtain licenses in requested states within 90 days of hire.
• Current licensure and certification must remain in good standing.

Nice to have

• Experience in cardiometabolic condition management, virtual care, centralized results management, or digital patient engagement.
• Experience with STARS/HEDIS measures, clinical comorbidity coding, and identifying care gaps.
• Experience with or strong interest in AI-enabled clinical workflows and clinical technology improvement.
