LPN Clinical Liaison

What you will do

• Document patient and member encounters in Thyme Box and supported electronic health records.
• Capture histories, assessments, treatment plans, orders, diagnostic results, referrals, prescriptions, and follow-up instructions.
• Prepare patient charts and collect clinical information, including vital signs and medication reconciliation, before visits.
• Verify member eligibility and insurance coverage, facilitate referrals and appointments, and keep care plans updated.
• Review charts across multiple electronic platforms to support transitions of care.
• Monitor shared clinical inboxes, triage messages, and coordinate communication with healthcare partners.

Requirements

• At least two years of LPN experience, especially in telehealth.
• Compact LPN license required.
• Ability to maintain patient confidentiality in accordance with HIPAA regulations.
• Strong organization, prioritization, listening, communication, and documentation skills.
• Comfort using or learning video chat, Google Suite, Slack, electronic health records, and other technologies.
• Ability to respond promptly to urgent member needs and work effectively in a fast-changing environment.
