Field Care Coordinator

Location: Boynton Beach, Florida, with candidates residing in the Boca Raton, Delray Beach, or Boynton Beach areas; requires 75–90% local travel to members' homes, assisted living facilities, long-term care facilities, and other care settings.

Salary: $53,700–$72,600 per year; current starting salary is $64,000–$68,000 based on qualifications and experience.

What you will do

• Visit Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings.
• Assess member needs and establish personalized care plans.
• Plan and implement interventions that support optimal wellness.
• Coordinate services with payers, providers, interdisciplinary teams, families, and community resources.
• Monitor and evaluate care plans against members' personal goals and health outcomes.
• Guide members and families toward appropriate care and social services.

Requirements

• Bachelor's degree in health services, social work, psychology, or a related field with at least 2 years of relevant experience; or Florida-licensed RN with 2+ years' experience; or Florida-licensed LPN with 4+ years' experience; or case worker with 6+ years of health services or case management experience.
• Prior experience working with Medicare and Medicaid recipients.
• Intermediate to advanced computer skills, including Microsoft Word, Excel, and Outlook.
• Valid state driver's license and proof of personal vehicle liability insurance required.
• AHCA fingerprinting and tuberculosis screening required for this patient-facing role.
• Availability to work Monday through Friday, 8:30 a.m.–5:00 p.m.

Nice to have

• Bilingual English and Spanish skills with the ability to pass a language proficiency exam.
• Community Health Worker experience or 701B certification.
• Nursing home diversion, long-term care case management, or geriatric population experience.
• Electronic case-note documentation and experience using multiple computer systems.
• Experience with health promotion, coaching, wellness, community health resources, or medication application.
