Case Manager- Taylor County

Location: Remote position supporting Taylor County and surrounding areas in Medford, Wisconsin; travel up to 40% for member visits, with occasional travel to hirify.global offices for training or meetings. Typical schedule: Monday-Friday, 8:00 am-4:30 pm CST.

Salary: $53,700-$72,600 per year.

What you will do

• Assess members’ health, safety, strengths, interests, and preferences and develop comprehensive Member Care Plans.
• Coordinate with Field Care Nurses to deliver services in the least restrictive environment defined by each care plan.
• Conduct face-to-face social assessments at enrollment and at least every six months, plus quarterly in-person home visits and monthly phone contact.
• Arrange social integration, community, employment, housing, and other non-medical support services.
• Evaluate risk factors, educate members, and ensure cost-effective service delivery.
• Maintain case notes, service authorizations, care plan updates, and other documentation.

Requirements

• Bachelor’s degree in human services or a related field plus at least one year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor’s degree in another field plus at least three years of relevant experience.
• Ability to travel up to 40% to visit members.
• Valid state driver’s license and personal vehicle liability insurance with limits of at least 100/300/100.
• Ability to work Monday-Friday during 8:00 am-4:30 pm CST hours.
• For home-based work, a dedicated workspace protecting member PHI/HIPAA information and internet service with at least 25 Mbps download and 10 Mbps upload speeds.
• Participation in hirify.global’s patient-facing TB screening program is required if selected.

• Residence within 45 minutes of the assigned coverage area.
