Medical Director, Musculoskeletal (MSK) Programs

Location: Remote USA

Salary: $280,000–$305,000 per year, plus annual performance bonus, equity options, and benefits.

What you will do

• Design the MSK care model, including intake, triage, treatment pathways, escalation to imaging or surgery, pain management, peri-operative optimization, and post-operative recovery.
• Build the agentic clinical AI system with product and engineering teams by encoding clinical reasoning, defining safety boundaries, reviewing transcripts, and refining member engagement workflows.
• Deliver care through a shared patient panel, serving as the clinical backstop for diagnostic ambiguity, red flags, and complex multimorbid cases.
• Hire, train, and lead Advanced Practice Providers, physical therapists, and care team members across multiple states.
• Develop quality measures and feedback loops covering function, pain, imaging and surgical referrals, opioid stewardship, and member experience.

Requirements

• Board certification in Physical Medicine and Rehabilitation, Sports Medicine, Rheumatology, Orthopedic Surgery, or Internal Medicine / Family Medicine with substantial MSK practice.
• Significant experience managing common MSK conditions in adults, including older adults with multimorbidity.
• Experience building or substantially redesigning a clinical program, pathway, or service line.
• Commitment to population-level quality measurement and outcome improvement.
• Experience serving diverse patient populations.
• Role requires practice in the United States while working remotely.

Nice to have

• 5+ years delivering MSK care in sports medicine, PM&R, or orthopedics.
• Experience supervising Advanced Practice Providers, physical therapists, or other clinicians.
• Experience building clinical software or AI-enabled workflows with product and engineering teams.
• Experience with virtual or hybrid care, Medicare Advantage, value-based care, peri-operative optimization, post-acute recovery, falls, or fracture prevention.
