TL;DR

Location: Must be based in the USA (Remote with occasional in-home visits in your local area).

Salary: $115,000 - $140,000 annually.

What you will do

  • Conduct primarily telehealth video visits with limited, occasional in-home visits.
  • Perform comprehensive clinical assessments focused on prevention, chronic condition support, and medication optimization.
  • Identify care gaps and contribute to individualized, whole-person care plans.
  • Collaborate with an interdisciplinary team including primary care providers, specialists, pharmacists, and nurses.
  • Utilize integrated technology and AI-enabled workflows to reduce administrative burden.
  • Foster trust and clear communication with members and their caregivers.

Requirements

  • Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license.
  • 3+ years of outpatient clinical practice in primary care, family medicine, internal medicine, or geriatrics.
  • Active BLS certification at time of hire.
  • Comfort with delivering care via video telehealth and using electronic documentation systems.
  • Must be willing to obtain additional state licenses within the first 90 days.

Nice to have

  • Familiarity with managed care models like STARS/HEDIS.
  • Experience performing preventive care visits with Medicare populations.
  • Experience supporting older adults with complex medical or social needs.
