Salary: $115,000 - $140,000 annually.

What you will do
- Conduct primarily telehealth video visits with limited, occasional in-home visits.
- Perform comprehensive clinical assessments focused on prevention, chronic condition support, and medication optimization.
- Identify care gaps and contribute to individualized, whole-person care plans.
- Collaborate with an interdisciplinary team including primary care providers, specialists, pharmacists, and nurses.
- Utilize integrated technology and AI-enabled workflows to reduce administrative burden.
- Foster trust and clear communication with members and their caregivers.
Requirements
- Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license.
- 3+ years of outpatient clinical practice in primary care, family medicine, internal medicine, or geriatrics.
- Active BLS certification at time of hire.
- Comfort with delivering care via video telehealth and using electronic documentation systems.
- Must be willing to obtain additional state licenses within the first 90 days.
Nice to have
- Familiarity with managed care models like STARS/HEDIS.
- Experience performing preventive care visits with Medicare populations.
- Experience supporting older adults with complex medical or social needs.
