What you will do

• Review caseload and prepare technology for virtual patient services.
• Navigate daily schedule using modern tools and partner with in-home Sprinter team members.
• Build connections with diverse patients while collecting information for services.
• Conduct health risk assessments, medication reviews, cognitive screenings, and provide health education.
• Collaborate on vital signs evaluation, blood draws, and patient-specific tasks.
• Coordinate preventive care, medication management, specialist referrals, and documentation.

Requirements

• Board Certified Family Nurse Practitioner licensed in TX, LA, and/or OK; Active license in California
• Pass national background check and valid clinical license verification
• Technologically savvy with laptops/mobile devices for charting and HIPAA-secure messaging
• Strong written and verbal communication skills
• Ability to work independently or in teams with professionalism and problem-solving
