Compensation: $110K - $115K

What you will do
- Review caseload and prepare technology for virtual patient services.
- Navigate daily schedule using modern tools and partner with in-home Sprinter team members.
- Build connections with diverse patients while collecting information for services.
- Conduct health risk assessments, medication reviews, cognitive screenings, and provide health education.
- Collaborate on vital signs evaluation, blood draws, and patient-specific tasks.
- Coordinate preventive care, medication management, specialist referrals, and documentation.
Requirements
- Board Certified Family Nurse Practitioner licensed in TX, LA, and/or OK; Active license in California
- Pass national background check and valid clinical license verification
- Technologically savvy with laptops/mobile devices for charting and HIPAA-secure messaging
- Strong written and verbal communication skills
- Ability to work independently or in teams with professionalism and problem-solving
