Location: South LA Center, 1005 E Rosecrans Ave., Compton, California 90221; travel between work sites or community locations and home visits as needed
Salary: $132,000β$146,000 annually

What you will do
- Provide physical therapy evaluation and treatment across acute, restorative, and maintenance levels of care.
- Assess participants, create individualized treatment plans, and coordinate care recommendations with the Interdisciplinary Team.
- Conduct home visits as needed and educate participants, caregivers, and family members on exercises, supports, and durable medical equipment.
- Document therapy services, participant responses, care plans, reports, and required department records.
- Support ordering, inventory, distribution, and maintenance of durable medical equipment.
- Oversee group maintenance exercises and contribute to program standardization, team education, quality assurance, safety, and infection control.
Requirements
- Bachelorβs degree in physical therapy.
- Completion of an approved physical therapy program accredited by APTA, the relevant allied health accreditation bodies, or another organization approved by CMS.
- Active California Physical Therapist license required.
- At least one year of experience working with frail or elderly populations.
- Ability to work in a fast-paced, evolving environment, adapt to ambiguity, seek feedback, and use data to support continuous improvement.
- Authorization to work in the United States is verified through E-Verify.
Nice to have
- Two or more years of practicing physical therapy experience.
- Bilingual Spanish and English; other languages are also valued.
- PACE, ADCH, or CBAS experience.
- State-issued driverβs license, personal transportation, and auto insurance as required by law.
