Location: 2420 Merced Street, San Leandro, California, United States; travel between work sites and community locations may be required.
Salary: $101,000–$116,000 annually, bonus eligible.

What you will do
- Lead and supervise the Social Work, Participant Advocacy, and Behavioral Health teams.
- Provide coaching, guidance, and performance management to direct reports.
- Oversee participant advocacy and enrollment, supporting trust, engagement, and transitions into the PACE program.
- Serve as the escalation point for complex participant concerns, grievances, behavioral issues, and social-service needs.
- Partner with the Interdisciplinary Team on assessments, care planning, interventions, and resolution of complex social and family dynamics.
- Coordinate staffing, scheduling, community partnerships, continuous improvement, and reviews of participants at risk for disenrollment.
Requirements
- Master’s degree in social work or equivalent education, licensure, and experience in social work, behavioral health, or a related human-services field.
- At least 2 years of experience leading, supervising, or overseeing social work, care management, behavioral health, or participant-facing teams.
- Knowledge of the psychosocial, behavioral, and advocacy needs of frail older adults and people with complex medical and social needs.
- Knowledge of community-based services, government programs, and external resources for older adults and caregivers.
- Strong judgment, communication, coaching, relationship-building, and interdisciplinary collaboration skills.
- Valid California driver’s license, reliable transportation, current auto insurance, medical clearance, and required immunization and occupational-health compliance.
Nice to have
- LCSW.
- Bilingual Spanish, Mandarin, or Cantonese skills.
- Knowledge of PACE, value-based care, integrated healthcare delivery, or highly regulated healthcare environments.
