Bilingual Telephonic Care Coach (Healthcare)

Location: Remote Illinois; occasional travel to hirify.global offices for training or meetings may be required.

Salary: $53,700–$72,600 per year.

What you will do

• Evaluate members’ needs and requirements to support optimal wellness outcomes.
• Coordinate appropriate care services, community resources, and support programs for members and families.
• Identify and resolve barriers that prevent effective care.
• Monitor patient progress through assessments, data, member conversations, and active care planning.
• Guide members toward resources that support their health and wellbeing.

Requirements

• Bilingual communication skills.
• Bachelor’s degree in a health and human services field.
• At least 2 years of related experience.
• Ability to use multiple electronic information applications and software programs.
• Ability to work remotely from Illinois with a dedicated workspace that protects member PHI and HIPAA information.
• Home internet with at least 25 Mbps download and 10 Mbps upload speeds.

Nice to have

• Experience working with Medicare and Medicaid recipients.
• Experience with electronic case-note documentation across multiple computer systems.
• Experience in health promotion, coaching, and wellness.
• Knowledge of community health, social service agencies, and community resources.
