Supervisor, Transitions of Care Coordinator (Healthcare)

What you will do

• Lead and support a virtual team of Transitions of Care Coordinators assisting patients moving home from hospitals or post-acute settings.
• Provide direct transitions-of-care patient support for approximately 30% of working time, including arranging appointments, transportation, supplies, and durable medical equipment.
• Coach coordinators, conduct one-on-ones, answer operational questions, monitor team channels, and manage performance across key indicators.
• Develop and implement workflows, job aids, training activities, and process improvements.
• Collaborate with TOC leadership, trainers, education teams, and cross-functional partners on program development and clinical model pilots.
• Support recruiting, onboarding, attendance monitoring, meetings, rounds, huddles, and regular performance reviews with senior management.

Requirements

• Medical Assistant certification; active or expired certifications are accepted and must be verified.
• Significant Medical Assistant experience may be considered in place of certification, with school diploma and transcript verification potentially required.
• At least 2 years of direct lead or supervisory experience in a virtual setting.
• Ability to provide patient support during transitions of care and coordinate healthcare-related services.
• Strong organization, attention to detail, communication, and technology skills.

Nice to have

• At least 2 years of healthcare administrative or office experience.
• Fully bilingual English and Spanish communication skills.
