What you will do

• Partner with providers before, during, and after home visits to support patient care.
• Welcome members, assist with translation, collect information, take vital signs, and answer questions.
• Perform phlebotomy and other medical procedures as directed by the provider.
• Close care gaps, review consent forms, and maintain accurate visit documentation in EHR systems.
• Coordinate outreach and visit scheduling while maintaining medical supplies and visit readiness.
• Drive independently or with providers to member homes.

Requirements

• Medical Assistant certification from an accredited program.
• At least one year of healthcare experience; home-based care experience is beneficial.
• Basic Life Support certification, or the ability to obtain it within 30 days of hire.
• Valid driver’s license and reliable transportation.
• Basic computer skills and comfort documenting in electronic medical records.
• Ability to walk up to three flights of stairs and carry up to 40 pounds of equipment.
