Salary: $70,000–$77,000 annually

What you will do

• Conduct thorough assessments of medical, behavioral, and complex social needs of pediatric patients.
• Identify and resolve barriers to care, including food insecurity, unstable housing, and transportation challenges.
• Collaborate with interdisciplinary teams to develop and maintain integrated care plans.
• Educate families on navigating local resources and agencies while providing emotional guidance.
• Build and maintain strategic relationships with community organizations serving medically complex children.
• Represent the organization at community events, health fairs, and resource meetings.

Requirements

• Masters’ degree in social work, community/social services, or case management.
• 2-3+ years of postgrad experience in social service case management.
• Minimum 1 year of experience working with the pediatric population.
• Knowledge and application of motivational interviewing techniques.
• Experience providing telehealth services and engaging families in a remote environment.
• Must be based in New Jersey.

Nice to have

• LMSW, CCMC, or C-SWCM license/certification (LMSW strongly preferred).
• Bilingual proficiency in Spanish.
• Comfortability working with acute populations and familiarity with school and community resources.
