What you will do

• Provide preventive and chronic illness care using a proprietary clinician-patient platform.
• Manage complex medical conditions including diabetes, hypertension, and mental health for patients of all ages.
• Travel monthly to onsite clinics in Texas to provide in-person clinical care.
• Collaborate with a multi-disciplinary clinical team to optimize patient outcomes and shared decision making.
• Deliver compassionate, empathetic care to a diverse and inclusive patient population.

Requirements

• Active board certification in Family Medicine.
• Active licensure in Texas (or ability to obtain it by start date).
• 5+ years of clinical experience in primary and chronic care.
• Proficiency with digital healthcare tools including Zoom, Slack, and Google Suite.
• Comfort with basic procedural skills.
• Physical ability to stand, walk, and lift up to 20 lbs for onsite visits.

• Professional fluency in Spanish, including cultural nuances for medical settings.
