• Conduct thorough health risk assessments and medication reviews.
• Collaborate with in-home clinical team members to evaluate vitals and arrange blood draws.
• Provide comprehensive care coordination, including preventive interventions and specialist referrals.
• Manage patient documentation and findings using HIPAA-secure messaging and charting tools.

Requirements

• Board Certification as a Family Nurse Practitioner.
• Active Family Nurse Practitioner License in Michigan.
• Proficiency with laptops, mobile devices, and HIPAA-secure communication apps.
• Strong written and verbal communication skills.
• Ability to work independently and pass a national background check.
