Salary: $23 - $28 per hour

What you will do

• Build relationships with patients and caregivers through direct, in-person re-engagement in assigned geographic areas
• Assist patients and caregivers with program re-enrollment, onboarding, and mobile app utilization
• Perform telephonic outreach to re-engage children who missed onboarding or have not utilized services
• Connect families to local community resources, social services, and healthcare providers to address SDOH needs
• Partner with Behavioral Health teams and local providers to support safe patient discharge planning
• Coordinate non-clinical care efforts, including pharmacy access and primary care provider connections

Requirements

• Bachelor's degree
• 3+ years of experience in a patient-facing hospital or facility role within a healthcare setting
• Valid driver's license and ability to travel frequently throughout South Missouri
• Ability to manage multiple tech platforms and work within an interdisciplinary team
• Capacity to navigate ambiguity and take initiative in an organized manner

Nice to have

• National Community Health Worker Certification
• Bilingual proficiency in Spanish
• Medical Interpreter Certification
