What you will do

• Provide clinical oversight and exceptional care to a cohort of complex patients via video telehealth visits.
• Lead a multidisciplinary team, including pharmacy, disease management, social work, and palliative teams.
• Collaborate with patients, caregivers, and community resources to empower patients toward health literacy and self-efficacy.
• Develop and standardize team-based community practice in coordination with clinical leadership.
• Utilize the proprietary Orinoco health information system and provide feedback for interface improvements.

Requirements

• Fellowship-trained and board-certified physician.
• Minimum of 3 years of clinical experience.
• Must be licensed to practice medicine in Tennessee.
• Strong clinical background in geriatrics and experience with complex chronic conditions in older adults.

Nice to have

• Prior experience with telehealth.
• Experience managing other clinicians.
• Multi-state medical licensure or willingness to obtain additional licenses.
