What you will do

• Conduct Health Home eligibility screenings, initial assessments, and periodic reassessments of patient needs.
• Develop and implement compliant care plans for patients with chronic diseases, including medical, mental health, and substance use conditions.
• Provide direct service to a caseload of chronically ill patients, facilitating referrals and connecting them to local community resources.
• Coordinate patient services with pharmacies, insurance companies, hospital discharge planners, and family caregivers.
• Perform crisis intervention and conduct home visits to ensure consistent patient contact and documentation.
• Maintain strict patient confidentiality in accordance with Health Home standards.

Requirements

• Associate's degree is a must.
• 2 years of Care Management experience.
• Ability to travel frequently to patients' homes in Brooklyn and Queens, NY.
• Strong working knowledge of local community resources and ability to handle crisis situations.
• Demonstrated ability to work with data reporting, documentation, and outcomes.
• Experience navigating data management systems, such as Salesforce.
