What you will do

• Conduct initial and ongoing health assessments for patients with chronic conditions.
• Develop and implement patient-centered care plans in collaboration with medical providers.
• Provide direct service and crisis intervention to a caseload of chronically ill patients.
• Coordinate care services with pharmacies, insurance companies, and family caregivers.
• Maintain accurate documentation of all services in accordance with Health Homes standards.
• Facilitate referrals and connect patients to local community resources.

Requirements

• Must be based in Buffalo, NY and able to travel to patients' homes in Erie County.
• Minimum of 1 year of experience in health homes, HARP, or case management.
• Associate's degree required; Bachelor's degree preferred.
• Proficiency in data management systems such as Salesforce.
• Strong communication skills and ability to handle crisis situations.
• Knowledge of local community resources.

Nice to have

• Bilingual in Spanish is strongly preferred.
• Master's degree.
• 3+ years of experience in Health Homes or HARP.
