What you will do

• Perform patient outreach during inpatient stays to introduce the TOC program and communicate follow-up plans.
• Schedule post-discharge follow-up appointments with primary care physicians and specialists.
• Coordinate with hospitals and provider offices to obtain necessary clinical documentation.
• Submit Home Care and DME orders and manage vendor follow-ups.
• Prepare patients for telehealth visits by assisting with technology setup.
• Coordinate and track lab testing and imaging results for clinician review.

Requirements

• Must be fully bilingual in English and Haitian Creole.
• Must be based in the USA.
• 2+ years of patient-facing and care coordination experience in healthcare, managed care, home health, or community health settings.
• Experience working within a telemedicine environment.

Nice to have

• Managed Care or Health Plan experience.
• Medical Assistant (MA) Certification (active or expired).
• Proficiency with electronic health records and Google Workspace (Chrome, Sheets).
• Experience easing patient stress and anxiety during difficult healthcare transitions.
