What you will do

• Lead Care Managers through day-to-day guidance, coaching, and performance feedback on complex patient issues.
• Carry a reduced caseload while modeling best practices in care management and patient engagement.
• Conduct audits of patient charts and UAS-NY assessments to ensure compliance with NYS and Lead Health Home requirements.
• Collaborate with referral and training teams to streamline member assignments and support staff development.
• Monitor productivity metrics to maintain a minimum 90% billable compliance standard.
• Manage crisis-related situations, advising on response steps and ensuring timely incident reporting.

Requirements

• Master’s degree in Social Work (required)
• 3+ years of Health Home Care Management experience (required)
• 3+ years of supervisory experience leading clinicians or Care Managers (required)
• Strong working knowledge of Health Home NYS requirements and Lead Health Home policies.
• Ability to travel in the field to support patients and accompany appointments.
• Proficiency in documentation systems such as Netsmart, Foothold, Salesforce, and UAS-NY.

Nice to have

• Prior leadership experience in a behavioral health or healthcare setting.
• Experience in the New York State Health Homes program.
• Bilingual Spanish proficiency.
