What you will do

• Introduce the care program to patients during inpatient stays and communicate discharge plans.
• Schedule post-discharge follow-up appointments with primary care and specialty providers.
• Obtain clinical documentation from hospitals and provider offices.
• Submit home care and DME orders while tracking vendor follow-ups.
• Assist patients with technology preparation for telehealth visits.
• Collaborate in clinical rounds to discuss patients at risk for readmission.

Requirements

• Must be based in the USA
• 2+ years of patient-facing care coordination experience in healthcare, managed care, or home health.
• Experience working in a telemedicine environment.
• Strong communication skills for interacting with doctors, nurses, and caregivers.
• Ability to work assigned shifts between 9:00 AM and 7:00 PM ET.
• Proficiency with electronic health records and computer platforms like Google Workspace.

Nice to have

• Managed Care or Health Plan experience.
• Medical Assistant (MA) certification.
