What you will do

• Develop and coordinate programs to improve total cost of care and medication management outcomes.
• Utilize data to proactively identify patients for population health interventions.
• Collaborate with providers and care teams to develop and execute drug therapy plans.
• Provide clinical consultation, drug information, and counseling to patients and healthcare professionals.
• Verify daily activities of pharmacy technicians and provide work direction to support personnel.
• Participate in the training and evaluation of pharmacy students and residents.

Requirements

• Bachelor of Science degree in Pharmacy from an accredited college.
• Current registered pharmacist license issued by the Michigan State Board of Pharmacy.
• Current controlled substance license issued by the Michigan State Board of Pharmacy.
• Knowledge of drug regimen review, pharmacy software, and clinical documentation.
• Ability to communicate effectively with practitioners and patients.

Nice to have

• Advanced training in at least one specialty clinical area.
• One year of experience in a hospital or retail pharmacy setting.
• Proficiency in Microsoft Word and Excel.
