Clinical Pharmacist (Medtech)

What you will do

• Conduct patient outreach and provide pharmacist services via inbound calls and asynchronous video consultations.
• Deliver MTM services, including comprehensive and targeted medication reviews and medication reconciliation.
• Assess medication therapy for unnecessary medications, inappropriate dosing, adverse drug reactions, and adherence barriers.
• Document clinical recommendations and coordinate with provider offices to ensure recommendations are enacted.
• Collaborate with Product and Experience teams to advise on platform enhancements to support patients and customers.
• Hire, train, and evaluate direct reports to optimize the provision of pharmacy services.

Requirements

• Active Pharmacist License with at least 3 years of pharmacy practice.
• Medical fluency in both English and Spanish.
• Eligibility to work in the United States (no visa sponsorship available).
• Ability to obtain licensure in additional US states as needed.
• Strong telephonic and interpersonal skills, particularly when serving elderly populations.
• Proficiency in Microsoft Teams, Outlook, Word, and Excel.

Nice to have

• Native fluency in Spanish.
• Experience working in digital health startups or with hospitals/health plans on quality improvement.
• Proficiency in Salesforce and other clinical documentation platforms.
• Previous experience in sales or call center environments.
