What you will do

• Conduct telephonic and video visits with patients and caregivers to build trust and strong relationships.
• Collaborate with interdisciplinary teams, including clinicians and nurses, to ensure patient-centered care for high-needs patients.
• Perform psychosocial screenings and develop personalized care plans tailored to individual needs.
• Navigate behavioral health, long-term care, and advance care planning.
• Identify and address social determinants of health and social drivers of utilization.
• Document all clinical interactions accurately within the electronic health record (EHR).

Requirements

• Master’s degree in social work and an active professional license.
• Minimum of 2 years of expertise in the medical field.
• Must be based in the USA.
• Ability to succeed in a work-from-home environment and proficiency with technology.
• Strong interpersonal and communication skills for remote patient interaction.

Nice to have

• Bilingual proficiency in English and Creole, Spanish, or Vietnamese.
• Experience in health insurance, specifically Medicare Advantage.
• Specialized experience in geriatrics.
