Salary: $70,000–$77,000 annually

What you will do
- Conduct thorough assessments of medical, behavioral, and complex social needs for pediatric patients.
- Identify and resolve barriers to care, including food insecurity, unstable housing, and insurance limitations.
- Collaborate with an interdisciplinary team of physicians, RNs, and therapists to establish individualized care goals.
- Develop and maintain integrated care plans addressing the holistic needs of the child and family.
- Build and maintain strategic relationships with community organizations serving medically complex populations.
- Advocate for family resources and represent the organization at community health events and resource meetings.
Requirements
- Masters’ degree in social work, community/social services, or case management.
- 2-3+ years experience in social service case management required.
- Minimum 1 year of experience working with pediatric population required.
- Must be based in the tri-state area (NY, NJ, CT, and/or PA).
- Experience providing telehealth services and engaging families in a remote environment.
- Knowledge of motivational interviewing practices and techniques.
Nice to have
- LMSW certification (strongly preferred).
- CCMC or C-SWCM license/certification.
- Bilingual Spanish proficiency.
- Comfortability working with acute populations.
