Salary: $70,000–$77,000 annually

What you will do

• Conduct thorough assessments of medical, behavioral, and complex social needs for pediatric patients.
• Identify and resolve barriers to care, including food insecurity, unstable housing, and insurance limitations.
• Collaborate with an interdisciplinary team of physicians, RNs, and therapists to establish individualized care goals.
• Develop and maintain integrated care plans addressing the holistic needs of the child and family.
• Build and maintain strategic relationships with community organizations serving medically complex populations.
• Advocate for family resources and represent the organization at community health events and resource meetings.

Requirements

• Masters’ degree in social work, community/social services, or case management.
• 2-3+ years experience in social service case management required.
• Minimum 1 year of experience working with pediatric population required.
• Must be based in the tri-state area (NY, NJ, CT, and/or PA).
• Experience providing telehealth services and engaging families in a remote environment.
• Knowledge of motivational interviewing practices and techniques.

Nice to have

• LMSW certification (strongly preferred).
• CCMC or C-SWCM license/certification.
• Bilingual Spanish proficiency.
• Comfortability working with acute populations.
