What you will do

• Lead the coordination of regionally aligned multidisciplinary teams to provide holistic care telephonically and digitally.
• Assess member health, psychosocial needs, and cultural preferences to develop and implement individualized care plans.
• Identify gaps in care and arrange necessary resources, including community, mental health, and financial support services.
• Advocate for members and provide education on health literacy, self-management, and medication plans.
• Monitor and evaluate care plan effectiveness, adjusting goals based on member progress and adherence.
• Coordinate discharge planning and post-transition care to ensure continuity of care between facilities and home.

Requirements

• Current, active, and unrestricted Registered Nurse license (Compact License) required.
• Nursing Diploma or Associate’s degree in nursing.
• 3 years of clinical nursing experience in acute, post-acute, or community settings.
• Certification in Case Management (CCM) required (or must be obtained within 18 months of hire).
• Must be based in GA, IN, KY, MA, MN, OH, PA, TN, VA, LA, or IA.

Nice to have

• Bachelor’s degree in nursing (BSN).
• Certification in Chronic Care Professional (CCP).
• Experience managing patients via digital channels, mobile applications, and messaging.
• Case management experience in a managed care setting.
