What you will do

• Collaborate with interdisciplinary teams, including nurses and dietitians, to deliver comprehensive virtual care.
• Support patients and families coping with chronic illnesses and complex psychosocial challenges.
• Educate patients and caregivers on kidney disease, dialysis transitions, and advance directives.
• Conduct virtual assessments to identify social, emotional, and financial needs focusing on social drivers of health.
• Develop and implement individualized care plans, connecting patients with community resources like housing and food programs.
• Manage referrals for high-risk patients and execute evidence-based interventions to remove barriers to treatment.

Requirements

• Master's degree in Social Work from an accredited program.
• Active unrestricted Social Worker license in the state of residence (USA).
• 1+ years of experience in social work, preferably in healthcare or community-based settings.
• 2+ years of experience delivering care to patients with CKD, ESKD, or other chronic diseases.
• Proficiency in virtual care platforms and knowledge of HIPAA regulations.
• Must be based in the United States.
