What you will do

• Screen members for Diabetes Clinic eligibility and appropriateness.
• Conduct detailed assessments and provide ongoing education on diabetes management pillars, including medications and lifestyle.
• Collaborate with nurse practitioners, social workers, and pharmacists to optimize medication regimens and care plans.
• Develop and track collaborative SMART goals with members to drive behavioral changes.
• Use motivational interviewing to influence positive, sustained health outcomes.
• Maintain detailed documentation of all member and provider interactions.

Requirements

• Must be located within one of the contiguous 50 states.
• Active unrestricted Compact Registered Nurse (RN) license or unrestricted Registered Dietitian license.
• Active Certified Diabetes Care and Education Specialist (CDCES) certification.
• Minimum of 2 years of clinical experience managing diabetes.
• Ability to obtain additional state licenses as needed.

Nice to have

• Experience managing diabetes in a remote setting.
• Proficiency with diabetes technology, such as CGMs and insulin pumps.
