What you will do

• Guide members moving from higher to lower levels of care, ensuring comprehensive and safe discharge plans.
• Collaborate with hospital care coordination teams providing remote and occasional on-site support.
• Assess clinical status, prior treatments, and anticipated outcomes to support effective care coordination.
• Develop and implement treatment plans with measurable goals for transitioning members to appropriate care levels.
• Monitor and evaluate available resources to promote quality, cost-effective health outcomes.

Requirements

• Active Registered Nurse (RN) license in Tennessee or a participating Nurse Licensure Compact state.
• Must be authorized to work in the USA; sponsorship is not available.
• Minimum 3 years of clinical experience.
• Minimum 5 years of experience within the healthcare industry.
• Proficiency in Microsoft Office and ability to communicate effectively via digital channels.
• Ability to pass Windows navigation, typing, and grammar assessments.

Nice to have

• Strong background in ICU, Transition of Care (TOC), Skilled Nursing Facilities (SNF), Long-Term Acute Care Hospitals (LTACH), Inpatient Rehabilitation, or Case Management.
