Location: Fully Remote - Must be based in Arizona and work Pacific Time (PST) hours

What you will do

• Provide virtual primary care visits, including comprehensive assessments, chronic disease management, and acute care interventions.
• Diagnose and manage chronic conditions such as hypertension, diabetes, and COPD.
• Order and interpret lab tests and imaging, coordinating necessary follow-up care.
• Manage medication prescriptions, titrations, and refills.
• Provide health coaching, lifestyle modification guidance, and patient education.
• Collaborate with multidisciplinary teams and provide feedback on the EHR platform to improve user experience.

Requirements

• Active Nurse Practitioner (APRN) or Physician Assistant (PA) license in good standing (Arizona).
• Ability to obtain licensure in multiple states within 90 days of hire.
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• Minimum 2 years of experience in primary care, urgent care, or internal medicine.
• Active BLS certification.
• Flexibility to work Pacific Time (PST) hours (Monday-Friday, 8:30 AM – 5:00 PM).

Nice to have

• Experience with value-based care models, population health management, and quality metrics (HEDIS, STAR ratings).
• Proficiency with digital documentation and telemedicine platforms/EHRs.
