Routine Primary Care APP (NP/PA) - Telehealth

Location: Fully remote within Arizona, United States. Pacific Time work hours are required, generally Monday-Friday 8:30 AM-5:00 PM PST; weekend shifts may be required in the future.

Salary: $120,000-$155,000 base annually, plus a performance-based bonus of $10,000-$20,000; total compensation range of $130,000-$170,000.

What you will do

• Provide virtual primary care visits, including comprehensive assessments, preventive screenings, chronic disease management, and acute care interventions.
• Diagnose and manage conditions such as hypertension, diabetes, COPD, and heart disease, creating individualized care plans.
• Evaluate acute concerns, order and interpret laboratory tests and imaging, and coordinate follow-up care, referrals, and home-based services.
• Manage prescriptions, medication titrations, refills, adherence, and patient safety.
• Provide health coaching, lifestyle guidance, and patient education through virtual care.
• Participate in case reviews, team huddles, interdisciplinary collaboration, and improvements to the electronic health platform.

Requirements

• Active Nurse Practitioner/APRN or Physician Assistant license in good standing and ability to obtain licensure in multiple states within 90 days of hire.
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• At least two years of primary care, urgent care, or internal medicine experience.
• Active BLS certification at the time of hire.
• Experience with telehealth or virtual care is preferred; flexibility for Pacific Time business hours is required.

Nice to have

• Experience managing chronic conditions, preventive care, and acute illnesses through telehealth.
• Familiarity with value-based care, population health management, HEDIS, and STAR ratings.
• Experience with EHRs, digital documentation, telemedicine platforms, and virtual care workflows.
