Nurse Practitioner (Remote, SC License Required)

Location: Remote within the United States; active South Carolina license required

Compensation: $75/hour regular rate; $85/hour premium rate

What you will do

• Provide phone- and video-based follow-up care for patients recently discharged from hospitals.
• Conduct telephonic outreach and scheduled virtual visits, including medication reconciliation and symptom screening.
• Order necessary follow-up labs, medications, and medical supplies.
• Coordinate care across hospitals, emergency departments, pharmacies, and community providers.
• Build patient trust, explain the care model, and connect patients with appropriate services.
• Collaborate with nursing and administrative colleagues while contributing to population health targets.

Requirements

• Certified Nurse Practitioner in adult, family, or geriatric care.
• Active South Carolina license required, with the ability to obtain additional state licenses.
• 5+ years of post-graduate clinical experience in primary care or geriatrics; two years may be substituted with skilled-nursing or long-term-care experience.
• Experience managing Medicare and Medicaid populations with complex, comorbid conditions.
• Motivational interviewing skills, strong communication abilities, and comfort delivering virtual care.
• Availability for 24–36 hours per week, including monthly weekend coverage and four-hour shifts across weekdays and weekends.

Nice to have

• Active licenses in Texas, Pennsylvania, Illinois, New York, Massachusetts, Minnesota, Louisiana, or New Jersey.
• Familiarity with Google Suite and Slack.
• Bilingual English and Spanish communication skills with cultural fluency.
