What you will do

• Provide telephonic outreach and scheduled video/phone visits for post-hospital discharge patients.
• Perform medication reconciliation, symptom screening, and order necessary follow-up labs or supplies.
• Build trust with patients, explaining the care model and connecting them to essential services.
• Coordinate care between hospitals, emergency departments, pharmacies, and community providers.
• Collaborate with the nursing team to meet individual and population health targets.

Requirements

• Active state licensure in PA (Required)
• Certification as a Nurse Practitioner in Adult, Family, or Geriatrics.
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics.
• Direct experience engaging Medicare and Medicaid populations with complex comorbid conditions.
• Proficiency in digital-first environments and virtual care delivery.
• Excellent written and verbal communication skills.

Nice to have

• Active licenses in TX, NJ, IL, NY, MA, MN, LA, and/or SC.
• Bilingual proficiency in English and Spanish.
• Familiarity with Google Suite and Slack.
