Nurse Practitioner (Medtech)

What you will do

• Conduct telephonic outreach and scheduled video visits for patients post-hospital discharge.
• Perform medication reconciliation, symptom screening, and order necessary follow-up labs or supplies.
• Build trust with patients to explain the care model and connect them to essential services.
• Coordinate care across hospitals, EDs, pharmacies, and community providers.
• Collaborate effectively with the nursing and administrative teams.
• Meet individual performance targets while contributing to population health goals.

Requirements

• Active Nurse Practitioner license in South Carolina (SC) required
• Certified Nurse Practitioner in Adult, Family, or Geriatrics
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
• Direct experience engaging Medicare and Medicaid populations with complex conditions
• Comfortable working in a digital-first, virtual care environment
• Must be available to work 32-36 hours/week, including weekends on a monthly basis

Nice to have

• Active licenses in TX, NJ, PA, IL, NY, MA, MN, or LA
• Bilingual in English and Spanish
• Experience with Google Suite and Slack
