What you will do
- Partner with members and families to identify needed supports and direct services toward goals for health, employment, and community living.
- Collaborate with clinical and social support colleagues to address physical health, behavioral health, and long-term service needs.
- Conduct thorough face-to-face visits to assess physical, behavioral, functional, psycho-social, financial, and employment/independent-living needs.
- Use authorization criteria to approve appropriate home and community-based services and confirm services are delivered and needs are met.
- Complete intake, enrollment, and ongoing assessments; develop and maintain Person-Centered Plans.
- Travel to members’ homes within the assigned area (mileage reimbursed) and work an 8:00am–5:00pm schedule (no on-call).
Requirements
- Location: Must live within Hamilton County or a surrounding county.
- Valid driver’s license and ability to travel to members’ homes for face-to-face visits.
- 2 years of experience in the healthcare industry or in an operations-type environment.
- RN preferred with Developmental Disabilities Nurses Association certification (CDDN for RNs or DDC for LPNs), or meet federal Qualified Developmental/Intellectual Disabilities Professional requirements (QDDP/QIDP), or have a related degree; alternatively, 5+ years as an independent Support Coordinator/case manager in a 1915(c) HCBS Waiver plus required workshop trainings.
- Proficiency in Microsoft Office (Outlook, Word, Excel, PowerPoint) and ability to complete a digital literacy assessment.
- Strong oral and written communication, interpersonal and organizational skills; ability to work independently with minimal supervision or as part of a team.
