Specialty Care Social Worker

What you will do

• Conduct telephonic and video visits with patients and caregivers to establish trust and support patient engagement.
• Perform social work interventions, including psychosocial screenings and personalized care plans tailored to patient needs.
• Provide emotional and caregiver support, navigate behavioral health needs, and support social determinants of health and utilization drivers.
• Collaborate with interdisciplinary teammates (clinicians, nurses, care coordinators) to ensure cohesive, patient-centered care for high-needs patients.
• Facilitate joint visits and participate in team huddles, panel reviews, and interdisciplinary team rounds.
• Document all clinical interactions in the electronic health record and coordinate with internal and local stakeholders.

Requirements

• Master’s degree in social work and an active license.
• Minimum 2 years of expertise in the medical field.
• Must be comfortable working in a fast-paced environment and with technology (including electronic health records).
• Comfortable delivering patient support over video and phone.
• Ability to work from home and manage multiple competing priorities.

Nice to have

• Bilingual in English and Creole, Spanish, or Vietnamese.
