Location: On-site in De Pere, WI (Physical presence mandatory 10:00 AM to 2:00 PM)

Salary: $22.00 - $24.00 per hour

What you will do

• Serve as the primary on-site contact for unannounced MAC field inspections and escort inspectors through the facility.
• Maintain facility readiness, including strict HIPAA compliance and the maintenance of the Readiness Binder.
• Facilitate patient enrollment into the Chronic Care Management (CCM) portal.
• Conduct daily outbound patient outreach via calls and texts to document health status and maintain engagement.
• Provide evidence-based health support resources, including exercise and meditation content, to patients.

Requirements

• Active LPN or LVN license in good standing in the state of WI.
• Minimum 3 years of experience as an LPN or LVN in a clinical office or remote care setting.
• Must be based in De Pere, WI, for on-site work 5 days per week.
• Proficiency with Zoom, Slack, and online patient enrollment platforms.
• Demonstrated commitment to HIPAA compliance and patient privacy.

Nice to have

• Background in healthcare administration, clinic operations, or front-desk management.
• Specialization experience in Orthopedics, Physical and Rehabilitation Medicine, Sports Medicine, or Neurology.
