What you will do

• Conduct virtual wellness visits, health risk assessments, and cognitive screenings.
• Provide comprehensive care coordination, including medication management and specialist referrals.
• Partner with in-home clinical team members to evaluate vital signs and coordinate blood draws.
• Document patient findings accurately using secure mobile technology.
• Educate patients on health and well-being to empower proactive care.

Requirements

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner License in California
• Credentialed Medicaid provider for the state of California (Medi-Cal)
• Proficiency with laptops, mobile devices, and HIPAA-secure messaging apps.
• Strong written and verbal communication skills.
• Ability to pass a national background check and clinical license verification.
