What you will do
- Engage with at least 25 high-risk members daily to complete assessments and implement personalized care plans.
- Coordinate with hospitals, specialty providers, and community resources to ensure seamless care delivery and avoid duplication of services.
- Advocate for members with healthcare professionals and support discharge planning from hospitals.
- Monitor treatment and medication adherence while providing health promotion and self-management training.
- Perform data analysis from internal systems to identify trends and assist in the preparation of Quality-of-Care reports.
- Develop and refine departmental policies and protocols using Quality Improvement principles.
Requirements
- Bachelor’s or Master’s Degree in Social Work, Nursing, or as a Certified Case Manager.
- 1-2 years of experience in clinical or community resource settings focusing on care coordination and case management.
- Fluency in both Spanish and English is required.
- Must be based in the US (PST timezone) and be able to drive to clinics and provide home health care.
- Minimum 1+ year of professional or personal experience supporting seniors, individuals with disabilities, or those with mental illnesses.
- Current CPR certification and TB test.

- Experience with health IT systems and reporting.
- Local knowledge of and connections to community healthcare and social welfare resources.
