Community Health Worker

What you will do

• Conduct face-to-face outreach to engage members in preventive health, maternity, and chronic condition initiatives.
• Educate members on preventive services and assist with Health Risk Assessments.
• Coordinate access to community resources, transportation, and financial support services.
• Collaborate with human services providers, community centers, and state agencies to support member needs.
• Represent the organization at community events and conduct training on preventive health initiatives.
• Refer members to care management departments for additional support beyond the scope of practice.

Requirements

• Must reside within 2 hours of Charleston, WV (Kanawha county).
• High School Diploma or GED.
• 1 year of experience in a healthcare-related customer service or marketing environment.
• Ability to travel 50% - 75% of the time for face-to-face member engagement.
• Demonstrated empathy, patience, and cultural competency when working with diverse populations.
• Strong problem-solving, communication, and multitasking skills.

Nice to have

• Background in basic medical terminology.
• Experience working with the targeted community or population.
• Previous experience as a Community Health Worker.
