What you will do

• Conduct virtual consultations and thorough assessments for patients with diabetes, interpreting lab and imaging data.
• Develop individualized treatment plans including medication management and lifestyle modifications.
• Collaborate with an interprofessional team of PCPs, endocrinologists, pharmacists, and social workers to ensure continuity of care.
• Serve as a clinical advisor and provide escalation support for specialty clinic staff.
• Utilize a proprietary electronic health information system and provide feedback for its improvement.
• Maintain patient records in compliance with telehealth regulations and legal guidelines.

Requirements

• Must be a licensed Nurse Practitioner (NP) or Physician Assistant (PA).
• 5+ years of outpatient clinical practice, ideally in endocrinology or diabetes.
• Minimum 2 years of experience specifically concentrated in diabetes management and education.
• Active RN/APRN or PA license and active BLS certification required at time of hire.
• Multi-state licensure is required.
• Proficiency in English and experience using telehealth technology/EHR.

Nice to have

• Spanish language proficiency.
• Prior experience in virtual care.
• Understanding of managed care, including STARS/HEDIS measures and clinical care gaps.
