Diabetes Advanced Practice Provider (NP/PA)

Location: Remote USA

Salary: $120,000–$155,000 base annually, plus a performance-based bonus of $10,000–$20,000, for a total compensation range of $130,000–$170,000.

What you will do

• Conduct comprehensive virtual assessments for patients with diabetes, order advanced diagnostics, interpret laboratory and imaging data, and develop individualized treatment plans.
• Manage medications, lifestyle modifications, monitoring recommendations, disease reviews, and quality-of-care interventions.
• Coordinate holistic care with PCPs, endocrinologists, APPs, pharmacists, nurses, social workers, diabetes educators, families, and caregivers.
• Provide clinical advisory and escalation support during business hours and participate in regular panel review discussions.
• Use the electronic health information system for virtual visits, maintain accurate records, and provide feedback to improve the platform.
• Participate in quality improvement, professional development, and compliance with telehealth, privacy, legal, and ethical standards.

Requirements

• Must be based in the United States and work remotely.
• Active and clear RN and APRN license or PA license, maintained throughout employment.
• Active BLS certification at the time of hire and throughout employment.
• Master’s or doctoral degree.
• At least 5 years of outpatient clinical practice experience, including at least 2 years focused on diabetes management and education.
• Proficiency in English, telehealth technology, and electronic health records; multi-state licensure and willingness to obtain additional licenses as requested.

Nice to have

• Experience in an endocrinology or diabetes-focused clinic.
• Virtual care experience and interest in continuing clinical nursing through virtual visits.
• Understanding of managed care, STARS/HEDIS measures, clinical coding, and care gaps.
• Spanish language skills.
