What you will do
- Conduct virtual consultations and thorough assessments for patients with diabetes, interpreting lab and imaging data.
- Develop individualized treatment plans including medication management and lifestyle modifications.
- Collaborate with an interprofessional team of PCPs, endocrinologists, pharmacists, and social workers to ensure continuity of care.
- Serve as a clinical advisor and provide escalation support for specialty clinic staff.
- Utilize a proprietary electronic health information system and provide feedback for its improvement.
- Maintain patient records in compliance with telehealth regulations and legal guidelines.
Requirements
- Must be a licensed Nurse Practitioner (NP) or Physician Assistant (PA).
- 5+ years of outpatient clinical practice, ideally in endocrinology or diabetes.
- Minimum 2 years of experience specifically concentrated in diabetes management and education.
- Active RN/APRN or PA license and active BLS certification required at time of hire.
- Multi-state licensure is required.
- Proficiency in English and experience using telehealth technology/EHR.
Nice to have
- Spanish language proficiency.
- Prior experience in virtual care.
- Understanding of managed care, including STARS/HEDIS measures and clinical care gaps.
