CareBridge Advance Practice Provider, Nurse Practitioner - Bilingual (Healthcare)

Location: New York, United States; primarily virtual with required in-person training and candidates expected to live within commuting distance of an office

Salary: $110,080–$198,144 annually. Sign-on bonus: $20,000.

What you will do

• Provide primary and urgent care to patients through telephone and telehealth video consultations.
• Develop and implement care and treatment plans for adults with chronic and complex medical and mental health conditions.
• Conduct patient histories, physical assessments, and diagnostic reviews as needed.
• Identify and close care gaps while addressing patients' physical and psychosocial needs.
• Educate patients and families about medications, nutrition, adherence, illness progression, and crisis prevention.
• Coordinate care with physicians, providers, families, and interdisciplinary clinical teams; maintain records and prescribe medication within state authority.

Requirements

• Bilingual or multilingual communication skills are required.
• Master of Science in Nursing.
• Active national nurse practitioner certification.
• Current, active, unrestricted Family or Adult Nurse Practitioner license in New York and an active RN license.
• At least 2 years of experience managing complex care cases.
• Experience with electronic medical records.

• Spanish bilingual proficiency.

• Experience supporting patients with developmental disabilities and chronic illnesses.
