Nurse Practitioner - Georgia

Location: Field - Georgia; territory coverage includes Savannah and Atlanta, with travel throughout the assigned service area required. The role combines in-home visits with telehealth care across Georgia markets.

Salary: $140,000–$165,000 USD per year, plus performance-based bonus eligibility.

What you will do

• Conduct in-home and telehealth visits for Medicare Advantage members.
• Perform comprehensive assessments, medication reviews, and chronic disease management.
• Develop individualized care plans addressing clinical needs and social determinants of health.
• Identify fall risks, food insecurity, caregiver challenges, treatment-adherence gaps, and other barriers to care.
• Collaborate with physicians, care coordinators, social workers, and other interdisciplinary professionals.
• Use Clover Assistant and EMR technology to monitor health, close care gaps, educate members and caregivers, and prevent avoidable hospitalizations.

Requirements

• Active Nurse Practitioner license in Georgia or ability to obtain licensure.
• Active DEA registration and national board certification as a Nurse Practitioner.
• Experience caring for adult or geriatric populations in primary care, complex care, chronic care management, transitional care, home-based care, or value-based care settings.
• Strong clinical assessment and chronic disease management skills.
• Excellent communication and collaboration skills.
• Comfort using technology and telehealth platforms.

Nice to have

• Experience in home-based care, house calls, home health, palliative care, geriatrics, or population health management.
• Experience managing complex and chronic medical conditions.
• Experience with telehealth technology and hybrid care delivery models.
