TL;DR

Location: Santa Clara, CA (Hybrid, full time; California within commuting distance of Santa Clara County)

Salary: $29.70–$31.00 per hour

What you will do

  • Conduct housing assessments to identify member preferences and barriers to tenancy
  • Develop individualized housing support plans and present housing options
  • Secure housing by completing applications, gathering documentation, and coordinating move details
  • Build relationships with local landlords and property managers and advocate on members’ behalf
  • Provide crisis intervention and long-term housing stability planning, including retention procedures and crisis plans
  • Perform health & safety home visits, monitor progress, and maintain detailed case documentation

Requirements

  • Bilingual English/Spanish required
  • 2+ years of experience in housing navigation or case management
  • Knowledge of local housing markets and housing assistance programs
  • Familiarity with housing regulations and tenant rights
  • Strong interpersonal and advocacy skills
  • Experience working with vulnerable populations

Nice to have

  • Bachelor’s degree in Social Work, Health Services, or related field
  • Experience with HMIS (Housing Management Information Systems)
  • Knowledge of trauma-informed care practices
  • Previous experience in healthcare or managed care settings
