What you will do

• Independently manage a caseload of complex, multi-need clients with long-term case management.
• Facilitate client engagement and readiness for treatment using Motivational Interviewing strategies.
• Perform brief risk assessments, safety planning, and de-escalation for clients in crisis.
• Coordinate care with Preferred Facility Network providers and health plans to ensure seamless access.
• Develop personalized care plans and monitor client progress using standardized clinical measures.
• Participate in multidisciplinary team meetings to present cases and contribute to program improvements.

Requirements

• Master’s degree in clinical social work or equivalent required.
• Active, independent clinical licensure (LCSW, LMFT, LPC, LPCC, etc.) is mandatory.
• Must be a full-time resident of the United States; work cannot be completed from outside the US.
• Significant experience managing high-risk clinical situations and navigating health plan procedures.
• Proficiency in case management and knowledge of community mental health resources.
• Ability to work independently in a remote environment while collaborating effectively within a team.

Nice to have

• Bilingual proficiency in Spanish is strongly preferred.
